I have my phone in a sleeve at work. a Krusell sleeve or phonepouch or whatever they are called.
Left pocket solo.
I don't carry much anyways, phone keys wallet and knife.
Droid razor M in otterbox, left front pocket.
iPhone 5s in otter box (thank you groupon), I used to put it in my pocket because I hate having things on my belt, but as my EDC has evolved I have given in to wearing my phone on my belt unless I'm wearing cargo pants or shorts.
**just checked groupon, otter boxes for 5 and 5 a only fifteen bucks!

Large Colonel Littleton phone case for my blackberry z10 with otterbox.
Got my S5 and other goods in a Maxpedition TC-2 and the flashlight in the attached mag pouch.
Just use the holster that came with my Otterbox ... iPhone 5

On the belt. Being a fairly large phone, the clip for the belt holster dug into my side, so I zip-tied some foam on it. Now I don't even notice it.
Galaxy note 2 in an otterbox. I broke 3 holsters the first month I had it. Now it's in a Maxped cpl radio holster. Only thing that'll hold my big phone worth a crap! Has same belt loops on back as all maxped sheaths & holsters. Will fit 1 3/4" belt horizontally quite nicely. Only thing I don't like is the bungee strap. I replaced it with a makeshift paracord strap when I bought it, & it's done fine ever since. Daily wear since October.
Otterbox holster clips suck. I've broken three and the whole unit comes unclipped easily. The Otterbox protects well thank goodness. Their customer service is great at replacing the crappy holsters. Got to get myself some kind of pouch that won't come off my belt. I do like the fact that the clip rotates.
I'm running out of excuses as to why I "forgot" it at home. I hate carrying a phone but the wife is at her whits ends with me and not carrying it.
Cargo pocket, the Otterbox holster, or in a Maxpedition Mini Pocket Organizer.
I keep my Otterbox wrapped Note 2 in my BRP or in the addon pouch on the front of my Moab 6.

My pocket EDC...Moto G in Otterbox Defender. I can get away with cargo type pants everyday- work or play. Also as I mentioned above- For now - I also like the Otterbox holster for belt carry - although I've since read enough about the belt clips breaking to make me think that the belt clip will most likely break sooner or later (in which case - in the pocket carry will do).
I saw this topic come back up and was about to shout FORKLIFT. But I see that I've already polluted this thread once, so I'll just shut up.

Like it a lot better than the otter box and it's lightweight I Do not use the defender because I have a tempered glass screen protector which makes the phone thicker thus it will not fit in the defender. I was cautious about using a lightweight case but they make a great mil spec lightweight
