How is it no one had mentioned the leatherman brewzer yet? That is what rides on my keys.
I also recommend the Ti Punisher bottle opener/ pry / wrench. its a fantastic tool for the cash. Use it alot more than my R.U.T & Anso. pry tip very sharp so rips thru' tape on boxes. carry mine in my Ti keysmart so i don't have to plead with the Mrs to get her sewing kit out to stitch up my pockets.
I received a package today with this little pry tool which I ordered online. You won't believe how chip it was.
It works as belt / pocket /money / key chain clip, bottle opener, pry bar.
I used it today to open boxes and even thou a longer pry bar might feel more comfortable to grab, it works fine and since its shorter and curved it's almost unnoticed in pocket or belt. I am glad I bought it.
It's kind of hard to slide, of course, it's a pry tool, but worked just fine.
I still want a thin, larger one though for my MT sheath.

That's neat. Looks like you could use it as a money clip, too, if you so desire.
You can check out the Gerber Shard for your MT sheath. It's cheap and does all of the things you listed. It's not huge, but big enough and serves its purpose well.
Those are abouts a whole dollar shipped from china They are awesome indeed, tho a bit tight for my liking. Also, they can rust so make sure you keep it dry-ish (and dont take it to the beach).
Use mine to open boxes all of the time. Great little tool.
You know since I posted that I've changed my setup. If the OP is still looking is be happy to give you my brewzer since its not being used.
